To request an official transcript, print and fill out the form below:

Request by Mail

Mail your completed transcript request along with appropriate payment ($5 per transcript).

Request by Fax

Fax your completed transcript request to 813.899.6772, then pay for your transcript request(s) online or call our office at 813.899.6706 to pay for your request by credit card (do not send your credit card information as part of the fax).
